Wow... That is one extreme combo!! I will seriously encourage you to rethink using DMT, or any psychedelics for that matter. With your psychiatric condition(s), you may be at high risk of developing further conditions from such use. Please step careful.
Nevertheless, the cause of your problems breaking through are undoubtedly the aripiprazole (Abilify). It acts as an antagonist at 5HT-2A receptors, which means that it binds to the receptor without activating it. The effect of DMT and other psychedelics is because they act as agonists at the same receptor, which means that they bind to the receptor and activates it.
Aripiprazole's action on these receptor is exactly the opposite of the psychedelics. When you are on aripiprazole, you are effectively blocking the ability of psychedelics to function, because the 5HT-2A receptors already have a substance bound to them. Aripiprazole is also the cause of your problems with MDMA, as it binds to the serotonin transporter just as the MDMA would do if nothing else was already there. This action effectively blocks or at least significantly reduce the efficiency of MDMA.
The clonazepam (Klonopin) is also likely to play a role, as benzodiazepines are known to reduce the efficiency of psychedelics. The other medications might also have something to do with your problems getting intense effects, but the real problem here is the aripiprazole and to a lesser extent the clonazepam.
Again, I urge you to rethink the use of psychedelics. You don't want to mess any more with your mind, it might lead to some very bad effects. The nature of a DMT breakthrough is hard enough to cope with for people without any psychiatric conditions. I would instead encourage you to seek psychiatric therapy (not just treating the symptoms with a cocktail of drugs) in order to hopefully do something about your condition(s).
If you find a good treatment program, you might be able to someday get off this terrible cocktail of intense drugs. But please do not stop taking any of them before consulting professional medical opinion from at least two different professionals.
