I was going to recommend the machine, but seems you are already on it. I personally can vouch for it, because 1) it's dirt cheap and easy to make 2) it works.
Regarding number of puffs - I think it is commonly agreed here, that it is better to try to consume the dose in as few hits as possible, ideally one hit. Don't have big delays between hits, like breath out, breath in some fresh air, breath out and go for next hit. Some even skip the fresh air part.
I can speak from personal experience that you don't even need 2nd hit most of the times (maybe just to kill the reamins of the dose in the bottle) and 3rd is quite challenging to do, you need someone helping you with it most of the times. Trick to lessening number of hits required is to control your inhale during the toke and do it as slow as possible, while maintaining the vapourization.
Regarding your previous use history - I simply don't know exact answer to this. I haven't been able to find strong proof of correlation between "not able to BT" and "drug use history" yet. It is jsut what I think might be related to DMT sensitivity. I think LSD is known to have the biggest potential for HPPD symptoms post-use among entheogens, but it is very individual. Some can take years of high doses and brush it off as nothing, others get residual tracers after couple of LSD experiences. It is also impossible to tell now whether drug you took was actual LSD or some of it's derivatives.
Regarding time needed for brain to recover - it might be more than months required. From anecdotal evidences, it might take a year or more to fully "recover" or get to previous "normal".
I can also offer following information - of course drug tolerance levels greatly differ between individuals, but for my circle the dose you needed to cross BT treshold was 25-30mg commonly. Some individuals were able to take more than 40 and stay awake, but these were more like exceptions, rather than average. I think I did slightly more than 40 on purpose just once and it was my last DMT trip
When you mentioned you tried 60mg - from personal experience, it is not easy to consume so much DMT typically, so might it be that you are burning some of the DMT? What does your smoke taste like, is it harsh/burning to the throat?
